Currently the new dialog to insert an image doesn't validate the input at all.
For example, if you enter nothing as filename and foo as size, you get
[[|foopx]].
The file name should be checked to be non-empty.
The field for the size should match /^\d*x?\d+$/ (perhaps after trimming
whitespace).
