What MW user does the refreshData effectively run as - Administrator or a "normal" user? I say this because I think the problems started after setting protection on the templates and properties. I'm using everything "out of the box" settings wise. Precise SMW version is r113227 I have seen this intermittently before usually after a SF template edit (I've posted on the list about it). I think a few others have seen it too - badon, for instance. This is the first time I've seen it do it consistently though.
