I checked the sources, and the problem reported by you - or similar to it - was fixed in https://www.mediawiki.org/wiki/Special:Code/MediaWiki/95706 (29 August 2011) I suggest you fully delete the old extension subdirectory and reinstall the 1.004 version from SVN.
