Instead of requiring the Skins to make this HTML themselves, standardise it
like we did for other components. Skins can style it exactly how they want with
CSS. But this mess has to stop, especially considering all the variations
aren't really justified. The variations are all minor implementation details
that could be straightened without breakage.
