Thanks for taking the time to report this!

I'm not convinced that this is a valid bug: I think you should have a linebreak
after finishing your header.

I can also reproduce the problem by entering the one line
== My title == something === Another title ===
on test2.wikipedia.org which runs MediaWiki version 1.21.

For future reference, testing with a supported version of MediaWiki first (e.g.
on test2.wikipedia.org) is highly welcome, as 1.16 is very old.
