Most likely there was some sort of transient issue, caused the db query to delete the image to fail (which MW did not handle so gracefully), but deletion of the page succeded. Huji undeleting and deleting the image probably worked to actually delete the image. Since this time, a lot of the filebackend code has also been re-written, and the original issue was a one-off (although such a situation should be detected/or the page and image deletion should be in the same transaction. Perhaps things have changed so MW handles that more robustly.). Hence closing worksforme.
