We should only render a book once for each file type.
Maybe this is a storage issue at the rendering server? If so, the rendered
book could be transferred to the requesting wiki.
Content will eventually become out-of-date, so we would need to define a
maximum cache age. Also, it would be helpful to provide a "purge" action to
force rerendering.
