Failed uploads simply generated a Failed message, and always allow another
upload attempt.
For invalid files that Commons won't accept (eg, if I try to upload an audio
file renamed as a .jpg), the app should return an indication that the file is
invalid, and not allow repeated upload attempts.
For connectivity and other issues, the fail reason should be noted if known.
