I haven't been able to reproduce this manually, but I've seen it enough times
to know it wasn't a freak incident.
Basically when a bot creates an item, two copies are created.

Note that both were created in the exact same second, and given back to back
numbers, which is usually impossible due to the high speed bots editing.
Additionally they are created with the same exact content, leading me to
believe that the software is creating the extra item.
