I already have one, but I often see this happening:

[09:23] --> spectie has joined this channel (~fran@***).
[09:23] <-- spectie has left this server (Changing host).
[09:23] --> spectie has joined this channel (~fran@unaffiliated/spectie).

I guess it happpens when the user /msg nickserv identify xxx after they joins
the channel, and the sequence is usually decided by their IRC client.
