This may be one or two unrelated bugs. Look at this picture on commons:

I uploaded new versions, some with the same resolution, some scaled down, but
they don't show up everywhere. The description page shows the old version, even
though some of the past versions are displayed correctly. Downloading the
current file gives an old version. Thumbs on the wiki show old or new versions
depending on their width (possibly due to png thumbs cached somewhere). Look
here for an example of inconsistent thumbnail behavior:
