The ULS icon is missing in random pages. Please check the links below for
screenshots:

I could trace this issue for same pages in Chrome / Windows and Chrome
incognito / Ubuntu. The issue occurs in random pages.
A missing settings icon will confuse the user and will give no chance for
typing in native language.
