This issue is still occurring while inserting image inside Media Settings and
Reference Dialog.
Steps to reproduce:
1.Click on a image inside VE
2.Go to Media Settings
3.Type something as caption and add another image inside it
Second way to reproduce:
1.Click on a reference inside VE
2.Type something and add a image in this dialog box
