I have not been able to reproduce this at en.wikipedia.beta, and I didn't want
to edit live category pages for testing.
In Firefox, I added a line of text. VisualEditor added a level 1 section
headign after it:
= <span dir="auto">Category:Medical and health organizations</span> =
