To allow for Squid/Varnish caching of the credit card form, we need to remove
dynamic elements from the form.
We need to add an AJAX method that will be fired on page load to set and
retrieve the contribution tracking id, rather than the current behavior which
generates a contribution_tracking_id in the extension the first time the page
is rendered for a user's session.
