I know its been marked duplicate, but I'll mention it here because the patten
that gets me it is completely different. This happens regularly (though not
always) when editing a talk page with a signature. I have made certain to be
very careful at time to click the button only once and it will still occur.
Substituted ones do not seem to be prone to this problem, at least to the point
that I've noticed.

As mentioned I'm using Firefox 5. I don't believe I have any custom javascript
running.
