I don't think that sending newsletters and invites to get active again is the same thing as sending notifications for changes that the user has decided to follow. The previous experience with notifications for user talk message shows that this annoyed mainly users of the English Wikipedia who don't edit other wikis and hence didn't know that email notifications existed in MediaWiki and thought it was something suspect or some big change. That said, I agree that the most sensible thing to start with is to enable the feature with the option disabled by the default for old users, but if possible enabled by default for new users (who won't experience problems).
