A reason field was added to the create account form when creating a new account
for someone else (if you're already logged in) (bug 21503).
This reason field is kind of confusing if you're not all that familiar with
creating accounts for others. There should be some sort of help text explaining
what the field is for.
The main problem is I have no idea what the help text should say, so I'm filing
a bug instead of fixing the issue ;)
