At one point my hunch was leaning toward something odd happening with the LinkCache during execution, as this is what is relied upon for the Title::exists check, and it looks like it is only cleared in specific cases in MediaWikiTestCase if cloned tables are used or if the page table is used, neither of which would happen if the test is not marked as in the Database group.
This reminds me of T150512#2814743 – it feels like this issue has the same cause (and I don't understand why that bug was closed: T150512#2885781)
