I just saw this on Prince Philip, Duke of Edinburgh (Q80976):

The aspect ratio is incorrect. It seems that the image on Commons was cropped a few days ago, and the image preview is now showing the new version of the image, but using its old dimensions. Presumably, the media viewer HTML is cached (including data-file-width="424" data-file-height="670" width="114" height="180" attributes) and we don’t purge it when a new version of the file is uploaded on Commons. A manual purge of the Wikidata page would probably fix this, but I haven’t tried it out yet.
