On cawiki we have a bot checking that all images used in lists have the correct monument template. It has been usefull to check new entries in the lists and to organize the images on Commons. I think this can solve your point 3)

This is the update direction from en Wikipedia to Commons 4) Check the en "unused photo holding page" periodically for updating manually the other way around (these are photo suggestions coming in from Commons to the en Wikipedia) Is this everything?
