Rename special page class and special page SpecialModifyBlacklist instead of SpecialArchiveBlacklist to avoid confusing. Add more HTML to the form. Comment: There are many blacklists in MediaWiki, so I think your original name of SpecialArchiveBlacklist makes more sense.
