A three-in-one phone line adapter is very uncommon.
I've never seen one at Radio Shack. I agree with Gaffer,
the first thing to do is see how the jack is actually wired
before a decision is made as to what's needed.
USA phone wiring doesn't use common connections, so
three lines would mean six active wires.

for certain, would be to remove the face plate and have a look at the
wires. Then if you have a spare phone you could check for "Dial Tone"
between the pairs. Just watch out for someone calling, the ringing
voltage can bite you. ;-)
I would suspect that there are six wires or posibly eight in there. A
bit like cat 5. However you can get three lines out of four wires.
One of them will be a common connection in this case.
