The problem seems to have resolved itself. I have now 2 entries in the combo
box labeled "show names from the"

These entries are (1) outlook address book and (2) contacts.

I did not do anything specific. I had gone to the mail applet in the windows
control panel and hit the "show profiles" button. I found only one profile.
I canceled out of the dialog. I cannot say for sure whether this fixed the
problem.

I can't explain how you have two instances of Contacts in the drop-down box
but only one shown when you try to change the Outlook Address book as I
suggested before.

All I have left is to delete your mail proflie and re-create it.   Control
Panel has a Mail applet, click Show Profiles, and click Remove on the
(should be) only profile.  Then click Add and re-create the profile.
Removing the profile does not remove saved mail or contacts or anything -
but just for insurance, make a copy of your PST file elsewhere first.
You'll also need to re-setup connections to ISP mailboxes and so forth.

The "full screen" and the "active window" are identical pictures.
PL] I do not know why that is happening. When I took the picture of the
active window, I had set focus to the combo box with the duplicate entries
for the contacts folder. Are you able to see the combo box labeled "show
names from the" If yes, then that is the combo box that has 2 entries; both
are called contacts.
