Okay, client is running outlook 2003 SP2, with 2 internet (pop3/smtp) 
accounts.

The original account was from his office, he still receives email on this 
account, but doesn't need to send out on it as he isn't really working for 
this company anymore. We'll call this account2

He setup a new account with his home adsl service and set it as the default. 
We'll call this account1.  When he sends a new email out, it comes out under 
account1, I reply to it, he then replies to my reply, outlook then says this 
message will be sent out under account2. The message shows up between to: and 
send buttons.

Both of which suggest an issue with smtp authentication on the various 
accounts and that if it fails on account1, outlook with silently use 
account2. For both accounts, smtp authentication is not required nor provided 
by the respective account providers.

I also tried the following from the first link

    To prevent outlook from sending out from the wrong account;
    goto Tools
    then 'Send recieve settings'
    then 'define send and recieve groups'
    click 'edit' button
    click on the account you don't want to send out from
    uncheck the 'send mail items' 

But still no joy.
