Well, the floppy is on /dev/fd0, and the boot image has already been copied,
and the file resides in /tmp.
It dies when copying the Windows installation files from CD. Everything
seems to work fine, and the CD drive spins and the disk "grinds" for about 5
minutes before getting to the error.

This sometimes happens when users have floppy devices that are not on
/dev/fd0 or /dev/fd1. If this pertains to you then the solution is to
make a disk image of the boot disk using dd (dd if=/dev/???
of=/tmp/disk.img) - where ??? is the floppy device. You can then point
the installer to use the disk image instead of the floppy drive to load
DOS.
