I have not tried iTunes 11 myself yet. But I've heard that it works quite well. The only problem I've read about is that the menubar don't speak when trying to access it by pressing the Alt key. But using Alt+f for File menu, Alt+V for view, Alt+s for store etc will make the menu items speak. This is the case with JAWS at least.
