Well, it isn't everything you asked for, but Russell Kroll's Switch Digger will do the first two items on your list. It takes a little configuration to tell it where your routers and switches, but works well once you have done the up front work. http://www.exploits.org/sdig/ It is open source, so you have access to the code. One imagines the rest of what you are looking for would not be too much of a leap.
