We had a similar problem with the WiSMs -- regardless of code version (we tried v3.2.150.10, v4.0.155.5 and v4.0.179.8).  For us, they would just disable and we had to shutdown and then renable the ports, and sometimes we would have to restart the entire modules.  Cisco confirmed that they have some "catastrophic" bugs with all the available versions of code that apparently only impact the WiSMs and only manifest under high-utilization conditions.  We were experiencing these crashes every few hours.

The Cisco engineers worked with us to capture the crash logs during the crashes so that the developers could identify the bugs.  The developers now say that they have identified the problem(s) and they expect to have a fixed version of the code available soon.  In the meantime, we are temporarily borrowing 4400 controllers until they released this fixed version of code.
