We're using Radiator. No real points of pain; what we appreciate are the features to manipulate requests and support all kinds of authentication methods and account backends, plus the amount of logging & debug options that it can offer. We run two servers to support wireless (TTLS/PAP & PEAP w/MS-CHAPv2), VPNs, authentication for various websites and MAC-based authentication for part of our wired network.
