I should have been more specific: there is no SIP-based linux solution that allows you to place calls from the SIP client endpoint and terminate the call onto the PSTN via a long-distance ITSP (like Net2Phone or DeltaThree). Of course there are SIP solutions like you described, and I have heard good things about the mitel product. Sorry to have not been more clear.
