M2/VoipInnovations includes it as part of each line for $1 or less depending on the tier you have. My average line including e911, cnam, and directory listing is just under $4. I know some companies want a $1500 setup fee, or an extra monthly fee, but they do not.
We have multiple termination providers like Gafachi, VoipJet, and a few others for failover or calculated rate pricing depending on the terminating DID.
