Make sure to address this in the notes when you ask for additional IP Space. Simply put - let them know that you have requested on (list the dates) for the IP Space to be swipped back to the original provider and they have failed to complete the task. Also note that you are worried about this causing issues w/ your new request. In short - it always helps to give them a heads up.
