With new firmware and a reset config I think it has to be your machine.
Nothing should persist after those two actions.
Have you tried a second computer? How about a mobile phone?

It also kind of sounds like you might have two devices with the same IP
(hence the login requirement again).

Try Chrome in Incognito mode, see if you can reproduce it
