I don't see anything wrong with the form but there are some things I would tighten up and then see what happens.
1. your pretty inconsistent with what you quote and what you don't I would put the rest of the quotes in. 2. I guess putting the result pages before all the actions works like a presentation action I just always put these into html files and then include them as presentation pages. > 3. I don't see and </BODY></HTML> Tags I think you might find it helpful to put them in I have had forms that work in one action without all the proper page tags but fail in another so I have become very careful to always include them.
