Here is one scenario I had-
I had a mail action problem because the relay restrictions on the mail server were set too tight. I had tighter restrictions on the mail server from the web server because of an upgrade and did not know it.
The taf page actually errored to the user because of the mail action failing because of the relay restriction. Sometimes the syntax of the relay allow can be a pain.
