Hi Granted there are a lot of email clients out there that render HTML, there are still a lot that don't. you should format and email using mime type multipart/alternative and have a plain text versions with a link to a pre-filled in form along with the html version of the form pre-filled in with there email client.
There is also another issue of sending personal information of an individual over a non-secure channel. This might sound like a convenience measure for the clients could become a boondoggle, because someone with a packets Sniffer on one of the routing points could submit the form for the individual and change information that essentially hi-jacks that persons identity.
