No, I have got some for several years now, got one just before sending
that mail. I think that happens if the network connection is down or if
the server-side session expires.

Should be rather easy to reproduce on any Wt-based website (I have seen
it on all of mine as well, but pointed out the project website because
it is an easier debug target), but it may require some specific
circumstances (too many concurrent users, temporarily disconnected
network, ...).

Using the debug mode for enabling error messages would be one option.
