It's the way that OLE Structured Storage files work. The way to shrink an MSI
back to its smallest size is to dump it to IDT files and reimportant them. If
you look, that's the way the WiX toolset works.
I have to ask though, "Why do you need to modify the MSI after the fact? What
can't WiX do for you?"

After an installer for our product is built, it may undergo a transformation by
a utility that we wrote...i.e. some properties are changed, Binary table
entries updated, and SummaryInformation updated. This tool uses the installer
COM API.
After this operation, the installer inexplicably grows from 9.9 MB to 11.7 MB
even though the elements being updated are simply being replaced. Before and
after should be approximately the same size.
But here's the real mystery:
If I open the new 11.7 MB installer in Orca, then close Orca without making any
changes or saving the file, the file shrinks back down to 9.9 MB.
Where is this mystery flab? Is there a way to get rid of it without using Orca?
