Raymond Chen had a great blog post about the basics of Start menu
highlighting a couple of years ago that shed some light on the topic.

All that said, my understanding is that it's still kind of a black box
to us installation developers. Something to check for though from his
points: Do the shortcuts point to applications that are already
installed on the machine? Like notepad or rundll32? Generally as long
as they point to an actually new file I've generally seen them show up
as "new".
