I have sorted my problem on the clean test machine.
FYI..
A missing reference to Extensibility.dll was causing the Addin not to load.
This was present on my dev box but not on the test box. i deployed the
"extensibility.dll" file alongside my addin dll in the application folder
and the addin loads correctly.
I am assuming based on other posts that this is the correct approach to take
but if anyone has any thoughts feel free to set me straight..
