Is this with IE7 and you get the generic icon with the folded corner in the upper right? If so, this is actually an IE7 bug. The icon in the resource file was referred to with an incorrect GUID in the registry. I'll see if I can dig up a link that describes it.
Problem, if it is the same, didn't happen with IE6 and I have not checked with IE8 yet.
