Logs don't show much, but I just figured it out. Actually had to do with the Dialog scheduling. Under certain conditions, the wrong dialog was loading--and it was disconnected from everything else.
Weird that it only showed itself on 2012 R2, but weird happens. Even weirder is no one in QA noticed the missing check box on the dialog. All fixed now.
