SOAPHeader.getElementDeclaration is returning null

The implementation of this method returns an instance variable, but this is
never set. It's probably old code that doesn't reflect the current derivation
model. The fix is to use the Element Declaration qname to retrieve from the
Description component the required ElementDeclaration. I'll fix this.
