Follow-up Comment #1:
I had reproduced nearly the same bug when deleting a team. In fact, the
problem is that menu actions were not executing directly (so you can
add/remove many times the same team)
Hope to have fix it by calling ActionHandler::ExecActions() after creating
each action.
