Try changing this:

Change this:

       $rows = $wpdb->get_results("SELECT ID " .

to this:

     $rows = $wpdb->get_results("SELECT ID " .
               "FROM $wpdb->posts " .
               "WHERE (post_status = 'publish') AND post_type = 'page' AND

               $inclusions, ARRAY_N);
