Some plugins offer static page caching (Super Cache, Total Cache, and Batcache, to name three), which would avoid a DB hit when the cache is primed. Plugins such as HyperDB can divide calls across multiple databases, database servers, or even data centers, and also across read/writes. That can help to ensure that write or admin calls are always prioritized, etc. As Dion pointed out, you're using a plugin that should probably never ever be run on a read-intensive site. That's your problem. "Who is online" plugins work better for something like forums and what not, which have a much more balanced read/write ratio.
