[WSG] accessible and stylish forms

I saw a web site that described how to create fully accessible forms and make them look purty.  Does anyone know where that would be?  I am having a heck of a time finding information on creating stylish and completely standards based forms.  The sad thing is, I did this already for a project at my last job but now I don't have access to those style sheets and coding.:(   Any help is much appreciated.
