Wow, I have to say I expected a short list but not as few as that. I know of course about JAWS but the pricing is quite prohibitive. It really is an area crying out for some open source input as in:

this is a good tool for getting a feel for how your pages are "heard" as
it is simpler/easier to use than full blown screen readers such as JAWS.
there is also a screen reader [outSPOKEN] for the mac which you can download a demo of, but i think it may have stopped being produced. http://www.synapseadaptive.com/alva/outspoken/outspoken_for_mac.htm
