To me the answer is simply that HTML is code, as is PHP, as is Perl etc.
CSS is also code.
"Markup" is a sub-set of "code": code intended to be rendered by a user agent, etc.
...and we can distinguish between that and the extra costs the agency adds by referring to the latter as "mark-up".
